Ask the custodian who holds the record, in writing
Hospital, clinic or practitioner.
PHIPA is the usual statute
It is on the Health Law shelf. Fees and refusals have statutory limits.
If they refuse, there is a complaints path to the IPC
That is not a court Form 14.
